QCG collects limited consumer health data through optional surveys related to specific services.
We do not collect detailed medical records, health conditions, or other identifiable health information beyond these minimal surveys with categorical responses. This data is collected only when you voluntarily complete such a survey as part of the matcher functionality.
B. Sources of Consumer Health Data
We collect this limited consumer health data directly from you when you submit survey responses via the Site.
C. Use of Consumer Health Data
We use this limited consumer health data to:
- Facilitate personalized callback recommendations as part of the matcher functionality.
- Share with your selected healthcare provider, only with your explicit consent via form -submission, to coordinate your inquiry or service.

Should our practices expand to include additional health data, we will update this Policy with full disclosures as required by MHMDA Section 4.
